Application forms - for calls for participants

The application form allows you to compare the suitability of applicants, based on the information given in the form. To support the selection process of the colleagues in sending countries, the application form should be clear, concise and ask only the questions that are relevant for making the preselection. 


AlertYou must leave out questions about sensitive information, such as disabilities, birth date, dietary needs, emergency contacts, passport information (in line with GDPR). You can ask for this information on a later stage from the selected participants only.

Cooperation platform already has a template for application forms. The template will include some questions ready. You can make use of an existing template and customise it to your call. You will find fom templates at this address: Webforms: Templates | TCA/NET Cooperation Platform

Please note: The forms are under construction until the Cooperation Platform will be connected to the SALTO website. For now, for any application form, please use 'Application Form Starter (Core Elements)' without deleting any of the existing question, but adding new ones, as needed.

When building your application form, think about what basic data you need in order to make a selection. Make sure you are covering all the relevant areas for information.

Start with basic questions:

  • First Name / Family Name

  • Gender (male, female, other)

  • Country of residence

  • Primary email/phone number

  • Name of the organisation

  • Address of the organisation

  • Applicant's role in the organisation (either open question or to be selected from a list of preferred roles)

  • Project number, OID, etc. if specifically needed


Content questions - some examples

  • Please describe the main activities of the organisation you are representing.

  • What is your experience in relation to the topics addressed in this event?

  • What is your motivation to attend this event?

  • How will you put into practice the competences you will develop in this event / the outcomes of this event?

  • How will this training benefit your work with young people / your organisation?

  • How did you find out about this call?


Take care of all the permissions you need for sharing participant data

  • I allow sharing of my name, organisation and email in this application to the other participants of the activity, the organising team, logistic coordinator and/other relevant people in the organisation of this activity. (tick box)

  • I commit in taking part for the full duration of the activity, including online pre-sessions, etc. (tick box)

  • I allow the National Agencies to use my email address for Research-Based Analysis of European Youth Programmes (RAY) https://www.researchyouth.net (tick box)

  • I am aware of the fact that I have to obtain health insurance and full travel insurance myself. (tick box)

  • I am 18 or older and the information in this application is correct. (tick box)

  • In the organisation of the event we will follow the principles of a safer space (confidentiality…). I commit to promote the creation of safer space with my own attitudes and behavior. (tick box)

When creating questions, consider
  • What data do you as organiser, trainers and/or facilitators really need?

  • Make sure your questions match with the aims and objectives, professional role, target group, etc. so that your colleagues can make a good preselection

  • Pay close attention of making a very clear participant profile, e.g. youth worker, mentor, volunteering organisation, researcher; make a limited choice in roles, if you want to make a good result.

  • You can create the application or get feedback for it with the rest of the project team, e.g. other NA / SALTO colleagues, trainers, facilitator.
